INJURIES & MEDICAL EMERGENCIES
Parents, legal guardians of minors, students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance activity conducted by the students before, during or after class time. This also extends to students taking class online via ZOOM. The undersigned gives permission to Portland Dance Center, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

ATTENDANCE & LATENESS
Good attendance is necessary to ensure maximum progress and enjoyment of class. The school reserves the right to have students who come more than 10 minutes late to sit out of the class. A minimum attendance standard will be required.

ATTIRE
Required dance attire, hair, and dance shoes must be worn to all classes. View our Dress Code for more information.

SAFER STUDIO POLICY
PDC requires all staff and students to stay home when they are ill. In order to reduce community spread of illness, we follow best practices and recommendations from our local health department and from our industry associations. PDC understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times. Therefore PDC focuses on a hierarchy of measures beginning with keeping ill people home, followed by frequent hand cleaning and good hygiene practices, amplified cleaning, and curricular and facility adaptations to minimize contact and mingling where possible. Please leave your child at home if they are unwell, have had a fever within the past 24 hours, have vomitted in the past 24 hours, and/or are coughing/sneezing profusely.  Makeup classes can be scheduled for missed classes.
